We offer a wide range of services designed to support individuals with developmental disabilities and their families at every stage of life.
RESIDENTIAL HABILITATION
Eligible individuals can access this service through the HCBS waiver, which helps them develop skills in-care, household management, finances, and socialization to promote independence in their communities.
INDEPENDENT HABILITATION
FAMILY SUPPORT
Independent Habilitation services provide training and support from Direct Support Professionals (DSPs) for under 24 hours daily, helping individuals improve self, socialization, and adaptive skills to foster independence and community participation.
FAMIY SUPPPORT SERVICES - IN HOME SUPPORT
Support for families with members who have developmental disabilities empowers caregivers, prevents out-home placements, maintains unity, and aids reunification.
FAMILY SUPPORT SERVICES - RESPITE
A short-term service provided to individuals with disabilities for the purpose of offeringtemporary relief to their primary caregivers. These services are designed to support family stability, reduce caregiver stress, and help maintain the individual in their homeand community.
EXTENDED HOME HEALTH CARE SERVICES
Home Health Care Services for individuals with Development Disabilities (DD) are available through the HCBS waiver those living with a caregiver. These services support staying at home longer.
DAYHAB
Our Day Habilitation services focus on promoting independence in activities of daily living, supporting personal health, and enhancing socialization skills.
PRE-VOCATIONAL REHABILITATION
We provide services to help individuals with disabilities, including those recovering from injuries enter or rejoin the workforce. Our offerings include skills training, career counseling, job searches, and consultations on job accommodations.
INDIVIDUAL EMPLOYMENT SUPPORT
A plan that outlines the nature and scope of rehabilitation, employment, and training services provided to an individual with a disability to support the achievement of their employment goals.
