EXTENDED PERSONAL CARE
Task that are nursing or medical in nature and are specific to the needs of an eligible individual.
Examples include compression wraps, medication assistance, nursing assessment, nursing visit, catheter care, deep wounds care, enemas, feeding tubes, foot care, lymphedema wraps, manual bowel extraction or bowel stimulation and medication set up.
RESIDENTIAL HABILITATION AND COMMUNITY SUPPORT
We help individuals live and take part in their community and support them with activities of daily living for up to 24 hours a day. We manage and administer client medications.
At Blossom, we develop client-centered individual programs to address client-specific needs and meet state requirements. Our caregivers are trained to meet the needs of each client.
AGENCY ADULT FOSTER HOME
Agency Adult Foster Home is a private residence in which adult foster care is provided by the owner or lessee thereof to four or fewer adults who are not related by blood or marriage to the owner or lessee.
HOMEMAKER
Intermittent, non-personal care tasks such as housekeeping, laundry and shopping.
PERSONAL CARE
Assistance with bathing, dressing, toileting, incontinence, medication assistance, transferring, mobility in the home, eating and personal hygiene.
RESPITE CARE
Temporary relief to the individual’s primary caregiver for a period of time.
TRANSPORTATION
The agency provides both non-medical transportation and escort services.
SUPERVISION
Services are for individuals who need to be watched due to their illness or impairment.
COMPANION
The agency provides essential assistance to the elderly and disabled adults in need of help with activities of daily living.
EPCS-Nursing & Nurse Education
This service may include nursing care to the extent permitted by state law that will maintain the health and well-being of the individual and allow the individual to remain in the community.
